Pairing up eHealth (Subsidies) Accounts of Elderly Couple
- Ask the elderly couple to produce their Hong Kong Identity Card or Certificate of Exemption for verifying their identities.
- Show or read to them the information sheet setting out the details on the declaration of spousal relationship, consent to share-use vouchers and the liability of making false statement.
- Invite the elderly couple to declare their spousal relationship and give consent to share-use vouchers (confirm by inserting their respective Hong Kong Identity Card into the Smart ID Card reader, or providing particulars for inputting in the eHealth System (Subsidies) if Smart ID Card reader cannot be used under exceptional circumstances).
- Ask the elderly couple to provide their respective SMS-enabled Hong Kong mobile phone number and choose the language of the SMS message.
- Verify that all actions relating to pairing up the two accounts have been completed and proceed to confirm the pairing up. SMS notification will be sent to the designated mobile phone numbers of the elderly couple once the pairing up is completed.
- If under exceptional circumstances that either party of the couple is unable to attend in person to declare spousal relationship and provide consent for pairing up accounts to share use vouchers, a duly completed “Consent of Sharing Health Care Vouchers with Spouse” attached with a copy of the latest Hong Kong Identity Card (or Certificate of Exemption, if applicable) of the elderly person who is unable to attend should be provided by the elderly person attending in person at the practice of a service provider for linking the accounts of the couple in the eHealth System (Subsidies)).
